What Records You Can Request
You have the right to obtain copies of any medical transport documentation created by Midwest Ambulance. Available records include:
- Trip reports and run sheets
- Facesheets and patient demographics
- History & Physical (H&P) summaries
- Medication administration logs (MOT)
- Billing and insurance documentation
To request patient records, please send an email to medrecs@midwestambulance.com
Attorney Request Process
Please submit this request via ChartSwap.com using the following steps:
- Register at http://www.chartswap.com/register as a Record Requestor
- Sign in and Search for a Provider, Enter Request Details, then Upload Supporting Documents
- Once your request has been reviewed and records are available, you will receive a notification and invoice which you can pay with a check or credit or debit card.
